Records, Licensing & Complaints

Effective May 11, 2026

Under Texas House Bill 4224 (89th Legislature, effective September 1, 2025), codified at Section 181.105 of the Texas Health and Safety Code, every Texas health care provider must post detailed instructions for consumers to request their health care records, contact the provider's licensing authority, and file a consumer complaint.

This page satisfies that requirement for Dr. Andrew Leo, DC, practicing at CoreHealth Wellness Center, 3500 Oak Lawn Ave., Suite 650, Dallas, TX 75219.

1. How to request your health care records

You have the right to inspect or receive a copy of your health care records. Records requests are processed by the front office at CoreHealth Wellness Center.

  • In person: Ask the front desk for a records request form on your next visit.

Please include in your request

  • Your full legal name (and any prior names used on your record)
  • Date of birth
  • Approximate dates of service
  • The specific records you'd like (e.g., visit notes, imaging, exam findings)
  • Where to send them (email, mail address, or another provider)
  • Your signature — Texas law requires written authorization for release

Response time. Texas law requires that records be furnished within 15 business days of receiving a written, signed request and any applicable fee.

Fees. Reasonable fees may apply as permitted by Texas law for copy, retrieval, and postage costs. The front office will quote the exact fee before processing your request.

2. Contact the licensing authority

Dr. Andrew Leo is licensed by the Texas Board of Chiropractic Examiners (TBCE). License number: 16795. The TBCE is the state agency that licenses chiropractors and investigates complaints related to chiropractic practice in Texas.

3. File a consumer complaint

You may file a consumer complaint with the Texas Attorney General's Consumer Protection Division, including concerns about how your protected health information was handled, as described in Texas Health and Safety Code § 181.103.

Federal HIPAA complaint

If you believe your health information privacy rights have been violated under HIPAA, you may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/hipaa/filing-a-complaint.
